Behold,
I will cast her into a sickbed, and those who commit adultery with her I will throw into great affliction,

Paraphrase
Watch! (Since she likes to be in bed,) I will cast her into a sickbed, afflicted and unable to get up, and those who commit adultery with her I will throw into great affliction, if they will not repent of her ways,

“Great affliction” is a phrase that can be translated “great tribulation.” This is one of two times Revelation uses this phrase. Notice that this use has nothing to do with the saints being persecuted for the cause of Christ, rather it is people being punished for wrongdoing.
